Forms
- BSA Health & Medical Record (Parts A & B) — Required for all Scouts. Includes the photo/media release. Updated annually.
- BSA Health & Medical Record (Parts A, B, & C) — Required for events lasting 72 hours or more (long-term camping, summer camp). Part C requires a physician's signature. Part C is only published as part of this combined form.
- BSA Activity Consent Form — Used only for specific activities that require additional consent beyond what's covered in Parts A & B. Your Den Leader will let you know if it's needed.
Need a Pack-specific form (registration packet, dues, etc.) that isn't here? Registered families can ask at a meeting or message a leader through Scoutbook. Prospective families can reach us through BeAScout.
