Forms
- BSA Health & Medical Record (Parts A & B) — Required for all Scouts. Updated annually.
- BSA Health & Medical Record (Part C) — Required for events lasting 72 hours or more (long-term camping, summer camp). Requires a physician's signature.
- BSA Activity Consent / Photo Release — Standard activity and authorization form.
